bronze cory bacterial infection?

I think I have a bronze cory with a bacterial infection, but I'm not completely sure.
1. Water parameters
a) Temerature range: 72-74 degrees F
b) pH=7.0
c) GH: soft
d) KH : low
e)Ammonia, Nitrate, Nitrite, are all zero
f) Water change frequency - 25% once a week

2. Tank set up
a) Size: 30 gallon
b) Substrate: thin layer of small gravel
c) Filtration: Penguin biowheel
d) Furnishings: plants, rocks, a cave or two
e) Other tank mates: 2 other bronze corys, 1 albino (not sure what species)
f) Set up for about a month

3. Symptoms / Problem description
There has been a general loss of color for a couple of days, and where the "bronze" usually is on the body, there is a white haze of some sort (not fluffy or raised up from what I can tell). The dorsal fin is also very ragged, and the eyes are a little sunken in. Since yesterday, she (I think) has become inactive except for short struggles to the top to get air then floats back down until she hits something. She tries to move sometimes but it seems as if she has lost all her energy.

4. Action taken (if any)
Since yesterday, I have taken the carbon out of the filter, added half a dose of Melafix twice a day (still a full dose, just not all at once like the bottle says...they seem agitated when I add it all at once). I have also added an extra airstone.
What really worries me is that I've had 2 other fish die with similar symptoms. If you read my last post, you know one was a peppered cory I had just introduced to the tank. The other was a panda cory in a separate quarantine (although his color didn't appear to change like the other two). They all had similar "loss of energy" symptoms with frequent struggles to the top. Sorry this is so long-winded, but I would like to find out what this is before it potentially takes out anymore fish. Thanks in advance.
