Would anyone be able to help me with this?

With the picture I can see it it not possible to assume anything that would...

What had you assumed is the cause of the hole?

Looks like the fish had been dead for sometime and something else in the tank just gnawed a hole in the carcass. I've seen it happen often.

This is the most likely. but could be mistaken for an internal problem, however in most casses this is not the case.
