Two males or an aggressive male can cause skin damage to a female during chronic fighting that can produce a similar look.
Have you ever noticed any fighting? If not then most of it happens when a female is not ready to spawn but somehow gets trapped within a cave for a few too many days by the male.
Based on my experience, Some L134 males entrap females inside the proposed spawning cave much longer than most other small Plecos. I have seen them kept inside a cave for up to 8-10 days and they sometimes emerge badly scraped up. Perhaps that is what has happened here?
If it isn't from fighting then there is a chance it has a disease. Might not hurt to remove it while it is inside a cave by moving the cave to a 10 gal quarantine tank and treating the water with methylene blue and a tsp of rock salt peer gal to promote healing. This is a gentle treatment but still effective when dealing with superficial abrasions. It is one of the rare times I use any salt.
