When Otos attack

Ok , not attack..but they attempt to suck slime off of other fish at times..which hasn't been a prob because the fish move.
But I have a Pearl Gourami that I'm watching because it seems to have scratched itself on something (maybe got stuck in rock or wood?)... and last night I noticed an Oto trying VERY hard to suck on the spot..leading me to further believe it is a wound of some kind..
What gives? I thought Otos were very peaceful lil guys ..
