I'd be hesitant to offer any thoughts as I can't say I saw something like this before. Try giving us more info. How long has it been like that? Where it could have come from - new fish or plants introduced into the fish tank? Any dynamics - has it been growing or reducing or never changing? Is it possibly a physical injury? External parasites? Was the fish treated against parasites ever?

So it must have come to you already bearing these areas of concern and we can't know anything about where, when and how it started. I am afraid I'll pass. Hopefully someone else will recognize what these manifestations might mean. You can also ask on Monster Fish Keepers in the Freshwater Diseases forum.
