Re: What is the disease?
Based on your picture, it looks like an intestinal infection. May have something to do with what it recently consumed.

Re: What is the disease?
The heavy swelling, and red patches are both indicative of an internal bacterial infection. I'm going to assume this isn't the same tank you used the tetracycline on. If not, I would advise giving it a try. If it is the same tank, it may be necessary to go with something like erytrhomycin to clear it.
A good tank cleaning prior to the dosing may also be helpful, but don't disturb the bacterial balance any more than necessary. Both gram negative and positive bacteria are effected by these meds, and it's important to at least have something to maintain the nitrogen cycle when the dosing is complete.
As to diet, I would be surprised if a fish in this condition will eat much. A lot of the swelling is inflammation of the intestine. Most foods are going to be too rough for them to want to try (think severely sore throat). Once the fish begin feeding again, a diet high in protein is not a problem. However, it should primarily be vegetable based protein. Ancistrus have an exceptionally long digestive tract compared to a lot of other species. This is evolved to break down plant based foods which aren't as easily digested. The problem of blockages generally occurs when a bolus of food is broken down early in the system and becomes impacted before reaching the lower intestine where moisture and nutrients would normally be removed prior to exiting the system. It's a very long intestine, but not all of it has the same purpose. Remember, we're dealing with critters that don't really have a true stomach.

Impossible only means that somebody hasn't done it correctly yet.

Re: What is the disease?
looks like bloat with a secondary bacterial infection
could be due to a high protein diet i have seen this
been brought about with feeding to much blood worm
water changes and fresh veg
