It seems to be the right one, LDA45 is the same as L349 Ancistrus sp from Peru and Rio Huallaga.
Maybe you can confirm if you ask your dealer if they are imported from Peru.
i asked in my shop,there was 3 imports:Brasil,Peru and Columbia.
He said he is 80% sure this one was on Peru shippment.

Jools i think you are correct, on closer inspection the head shape does seem more like that of a Lasiancistrus, especially looking at the last photo posted - as such i am somewhat stumped as to the identity of the fish! very pretty though.
