Funny - i had a tiny sense that maybe it could have come with the blood worms which would have been the only "new" thing that I added to the tank recently.

what do you mean by filtered tap water? why was simple tap water not good enough? (from what you are saying the tap water is like RO water but with extra fluoride?) And you didn't speak about your water changing regimen or what you do when you clean your filters? There is such a thing as 'cleaning too much', but food on the bottom for more than 15mins is generally not good.
